If you do not have a referral form, you can use mine: Click here to download my referral form. I will reach out to the referred patient/client, ideally within 48 hours of receiving a referral.
Be sure to first ask patients/clients you would like to refer to my counseling practice if they are willing to be referred. If they express interest, seek their permission to share the relevant personal information with my practice, and to allow me to contact them to explore their mental health care options.
